WebPlus X6: Form Combo boxes not appearing

Can anyone assist please.

I'm trying to set up a simple form to order goods from a menu. (This is a test one at present).




However when the form is submitted the quantity (combo) boxes for the panini and coffee do not appear?

I've attached images of what is displayed.
